A Family Health Team (FHT) is an innovative approach to heath care, and one that was implemented by the Ministry of Health and Long Term Care across Ontario in 2005.

Family Health Teams allow physicians and other allied health providers to work as a team to focus on keeping patients healthy. The vision calls for physicians, nurse practitioners and other members of the team to practice together in a positive working environment, sharing and benefiting from the complementary knowledge and skills of their colleagues.”

Simply put, an FHT provides an individual with ongoing health care through a comprehensive network of health care providers. This network is comprised of family doctors, nurse practitioners, registered nurses, dietitians, social workers, pharmacists, and health educators, and is created based on local health and community needs. A sample of the health care issues that a FHT manages includes health maintenance, chronic disease management, disease prevention, health promotion, and preventative medicine.
